Using RealMe® verified identity is the quickest and simplest way to prove your identity to us, but there are other ways to meet this requirement.
In the first instance, if you are local to Albany or Auckland, we may ask you to visit our offices with your formal identification so that we can verify it and then progress your panel provider application. Please call our office on 09-9129100 to make an appointment.
If you are outside of the Auckland area, we would ask that you please visit a Justice of Peace and request certified copies of your identity documentation, along with a signed document from them confirming your likeness to the photographic identification which you have provided. Any formally accepted NZ photographic identity documentation is sufficient; a NZ driving license, NZ Passport or NZ Firearms license.

You will need to provide a covering letter with the details of where you are based, and what formal role you hold within the NZ Heath Sector, detailing what Regulatory Authority your current practising certificate is issued under and details of your current Scope of Practise.
Once we have received your documentation we will then verify them with your Regulatory Authority and contact you to discuss the services which you wish to provide.
