The first instance of this system is currently running in Northland across their Primary Mental Health Programme which is a set of services consumed from within Primary Care, coordinated by PHO based regional coordinators and delivered by external service providers.
This has been a great success. The model and product is easily modified to cater for almost any type of services, it is not specific to Mental Health.
This product brings together the Primary Care providers and the DHB funded midwifery services. Recently, routine prenatal care has been pushed back into Primary Care, this system helps to support the delivery of funding and coordination between primary and secondary services.
